Climb – Champion Manager 1 On-Line Manager Survey Form

Thank you for taking the time to provide feedback for your teammate. The answers you provide will be given to them after they start the course. Within the first week, they will be setting up a meeting with you to go over the results and start working on key areas that they can improve and key areas that they can continue to do a great job. Please answer honestly and explain your answer so that they know specifically what you mean and how to improve.

An easy format to use in the description is to first describe the behavior that is good or needs improvement. Then the impact that the behavior has on you, the team, or work environment, and finally a suggestion as to what they could continue to do or to do differently.

Example:

  • When you answer the phone I noticed that you aren’t smiling much and seem to me to be frustrated. That could impact the client in a negative way and may give you a tougher call. Moving forward would you be willing to smile and be more positive with the clients?
